Prior to the appointment the employee or member must obtain documentation from their treating physician. Please know an appointment will not be conducted until the following documents are received and reviewed by the Specialty Medication Support Program practitioner:
- Recent progress notes (visit summary(ies))
- List of all current medications and dosage
- List of all medication allergies
- Recent blood work – for diabetics this should include blood sugar and A1C results
The treating physician’s office should fax this information to the Specialty Medication Program at 215-938-2154. The records should include the patient’s name and date of birth.
Additionally, the treating physician’s office should send the prescription via electronic prescription to EasyRx, or by fax to EasyRx at 215-544-5858, or by calling EasyRx at 1-833-327-9791.
