For individuals recovering from an illness or surgery, Special Recovery at Home provides that extra support needed to continue healing in the comfort of familiar surroundings.  

Home-Based Care

Redeemer Health provides recovery support to help you after surgery or if you have chronic, disabling diseases or conditions. Our occupational therapists, physical therapists, and speech-language pathologists can help you recover from these conditions and live more comfortably and productively at home. Home health care services are usually covered by Medicare and most private insurers under “Home Health Benefits.”

Reasons You May Need Help Rehabilitating at Home

After a procedure, such as joint replacement, heart surgery, or cancer treatment, you may require additional ongoing therapies. Or you may have a chronic condition or disability, trouble seeing or hearing, or have other reasons for needing help with health-related tasks. Whatever the reason, Redeemer Health's compassionate in-home rehab specialists provide the instruction, therapy, and treatments you need in the comfort of your own home. 

How Special Recovery at Home Works

As part of your initial assessment, a member of Redeemer Health's Home Care team will visit you at home or in the hospital to determine your needs, which may include skilled care, such as nursing or therapy, and talk with you about your health goals and concerns. 

Based on this initial assessment, we will work with you, your loved ones, and your doctor to develop a step-by-step plan of care. This program is designed to allow you to rehabilitate, recuperate, and continue living independently as you work toward your health goals.  

Redeemer Health's In-Home Therapy

Therapists evaluate your needs and develop a customized program to fit your goals and abilities. Assessing your needs within your home provides greater insight into your needs within your own environment. 

Redeemer Health's home rehab services include:

  • Occupational therapy to help patients achieve and maintain independence in daily activities
  • Physical therapy to improve and restore mobility, movement, strength, flexibility, balance, and coordination
  • Speech-language therapy to overcome problems speaking or communicating

Redeemer Health supports the community with special recovery at home in New Jersey, including Union, Somerset, Mercer, Cape May, Cumberland, Camden, Ocean, Gloucester, Middlesex, Salem, and Atlantic counties, and Pennsylvania, including Montgomery, Bucks, and Philadelphia counties.
