Medical Records

Requesting Your Medical Records is as Easy as 1-2-3!

Complete a request from your smartphone, tablet or computer -- anytime from anywhere. 
 

  1. Click here to request your medical records
    • Select "For Patients" from the top menu 
    • Select "Medical Records" 
    • Select CLICK HERE under Request Online Option
    • Be sure to have your valid driver's license/Government ID available 
       
  2. Complete Online Request: You'll be guided through every step of the process. Note: Federal and state laws allow a fee to be charged for the copying of patient records. 

 

  1. Review + Sign + Submit Request

Your records will be delivered directly to you!

Electronic Delivery

Postal Mail Delivery

  • You'll get an email with instructions for retrieving your records. 
  • To keep them secure, you'll need a PIN that is sent in a separate email.
  • Allow at least two business days for your electronic records to arrive.
  • Records will be mailed to the address you entered in the request.
  • Delivery time will vary based on your local post office.
