Our Services

Our heart care specialists at Redeemer Health Cardiology evaluate symptoms such as chest pain, palpitations, shortness of breath, dizziness, lightheadedness, fatigue, and leg swelling to diagnose and treat high blood pressure (hypertension), high cholesterol (hyperlipidemia), heart valve issues, coronary artery disease, and congestive heart failure. At Redeemer Health Cardiology, we want you to be empowered about your own health. That’s why we’ve created the Patient Portal—a secure place where you can view your test results and health records, as well as schedule appointments.

If you’re a new patient or haven’t received your Patient Portal logon information yet, be sure to get it at your next appointment.


Prescriptions

For prescription refills, please submit your request through the Patient Portal or call our office or have your pharmacy call our office. Please allow at least 24 hours for the prescription to be called in. You’ll need the following information:

  • Your name
  • Your date of birth
  • Your phone number
  • Name of medication, dosage (30- or 90-day supply)
  • Pharmacy name and number (or mail-order pharmacy)

 

Patient Forms

Welcome to Redeemer Health Cardiology. As a new patient, you will be required to complete some forms before your appointment. These forms will be mailed to you. In order to make your first appointment go as quickly and smoothly as possible, please complete all forms and bring them with you to your first appointment along with your most recent insurance card, driver’s license or other photo ID and your co-payment. If you have copies of recent test results or a disc of any radiology studies that were NOT performed at Redeemer Health, please bring them as well. If you’re a new patient, please arrive 15 minutes before your scheduled appointment.

Please note: We need the actual insurance card in order to scan it into the system. We cannot scan a paper from the insurance company stating coverage or a picture of an insurance card from a cell phone.

 

Payment/copay is expected at the time of your appointment. We accept cash, check, VISA, MasterCard and Discover.
